Discharge Planning
Discharge planning often begins before you are admitted. During your hospital stay, your care and care needs are reviewed by a nurse case manager and, when indicated, a social worker from
Care Management Department. The case manager maintains contact with your insurance company to discuss your treatment and care needs and to provide your insurance company with the information necessary for you to utilize your health insurance benefits. The social worker will help you with planning for your care needs upon discharge and arranging appropriate post-dscharge services.

Post-Discharge Assistance

If you anticipate that you will need assistance upon discharge please ask your nurse or doctor to see the social worker as soon as possible. You may also contact the Care Management Departent at 908-685-2953 to be linked with the social worker that can best assist you. The social worker can assist you with financial resources related to your healthcare needs, long-term care placement, homecare services, acute and sub-acute rehabilitation, hospice and palliative care services. The social workeres in the Care Management Department are also knowledgeable about various community resources that may be of benefit to you and can offer support as you cope with healthcare concerns. 

Hospice Program
Hospice provides an alternative for terminally ill patients who choose to remain home during the final phases of their illness, cared for by family and friends with the support of a team of medical professionals, clergy, social workers and trained volunteers. Somerset Medical Center works collaboratively with Somerset Hills Hospice, the hospice program of the Visiting Nurse Association of Somerset Hills, to provide hospice services for medical center patients.
